- A = name of street
- B = House Number (usually blank in rural areas)
- 1 = Dwelling House Number in Order of Visit
- 2 = Family Number in Order of Visit
- 3a = Last Name (Surname)
- 3b = Given name(s)
- 4 = race (W = white, B = Black, M = Mulatto, C = China, I = Indian)
- 5 = M (Male) or F (Female)
- 6 = Age at last birthday PRIOR to June 1, 1880
- 7 = Birth month (if the baby is listed less than 1 year of age)
- 8 = Relationship of each person to the head of this family (wife, son, daughter, servant, boarder or other)
- 9 = Single (if marked)
- 10 = Married (if marked)
- 11 = Widowed or Divorced (W or D)
- 12 = Married during the year prior to June 1, 1880 (if marked)
- 13 = Occupation
- 14 = Months unemployed (prior to June 1, 1880)
- 15 = Sick or Temporarily disability at time of the visit, nature of sickness or disability
- 16 = Blind (if marked))
- 17 = Deaf and Dumb (if marked)
- 18 = Idiotic (if marked)
- 19 = Insane (if marked)
- 20 = Maimed, Crippled, Bedridden or otherwise disabled (if marked)
- 21 = Attended school within the census year (the year prior to June 1, 1880)
- 22 = Cannot read (if marked)
- 23 = Cannot write (if marked)
- 24 = Place of birth, naming State or Territory of US or Country, if of foreign birth
- 25 = Place of birth, of father
- 26 - Place of birth, of mother
